The client must consent to full participation in the therapy process as detailed below.
• Listening to the audio between sessions is a vital element in the process and participation is required, failure may result in the therapy being less effective.
In the event of the client being unable to attend clinic in person due to severe weather, colds and flu, travel problems etc, online or telephone consultations will be used to avoid delay in therapy.
• Hypnotherapy is an effective method of treatment; however, success is not guaranteed and will only work with the client's full cooperation and the client must acknowledge this. Time scale and results may vary from person to person.
HYPNOTHERAPY IS NOT RECOMMENDED FOR THE FOLLOWING CONDITIONS (PRESENT OR PAST) Psychosis or Delusional Disorders, Personality Disorder/BPD, Anorexia Nervosa, Schizophrenia. By signing this form you confirm you have not previously suffered or do not currently suffer from any condition from this list.
• Should the client turn up to an appointment under the use of alcohol or drugs or is abusive, We reserve the right to cancel the session and the session will still be charged for.
• We will only use the details above with your consent, unless it is thought you are at risk to yourself or to others, or you were taken ill during your session, in which case your next of kin and/or medical assistance will be contacted. Confidentiality may also be broken if a criminal act is disclosed.

• We assure all client information is kept strictly confidential from the initial approach, treatment and thereafter. The written records of your treatment are stored safely within GDPR guidelines, full details are displayed in the privacy policy at www.believeachieve.uk
• Cancellations: 24 hours notice of cancellation is required, otherwise you will still be charged.
• In the event of my death or serious illness, your details/records will be available to a responsible person to advise and help you.
